Acne scars are a common problem that many people face during their adolescent or adult years. They are the result of skin damage caused by acne lesions, which leave behind scarring tissue as they heal. Acne scars can manifest in different forms, such as depressions, raised areas, or changes in pigmentation, and often have a negative impact on appearance and self-confidence.
Acne scars not only cause dissatisfaction with one’s appearance, but can also lead to social difficulties and psychological stress. They are a frequent source of frustration and concern for those who struggle with them.
Although acne scarring can cause a variety of problems for individuals, there are now methods available to help improve the appearance and texture of scars. Subcision is a technique commonly used for the treatment of acne scars.
The principle behind subcision is to use a specially modified sharp needle and blunt cannula to pierce through the skin surface and reach the base of the scar tissue. Through a fanning, multi-puncture technique, this method disrupts and loosens the fibrous scar tissue that is pulling down on the epidermis.
This process cuts and separates the fibrous scar tissue anchoring the epidermis downward, allowing the skin surface to return to a more normal position rather than being persistently pulled down.As the tissue then undergoes repair and new collagen production, the appearance of the depressed scar can be improved.
Yan’s Group currently utilizes an advanced subcision method, which involves the use of a blunt cannula needle. Through a fanning, multi-puncture technique, combined with a threading needle approach, this modified technique can reduce the occurrence of subcutaneous bleeding while still effectively cutting and separating the scar tissue that is pulling down on the epidermis.
This helps to significantly shorten the bruising and recovery time, while maintaining the same therapeutic benefits.
Subcision is not considered a surgical procedure, as it does not require incisions or sutures. The outward appearance will only show a few small needle punctures. It is typically performed under local anesthesia or nerve block, and aside from some initial discomfort during the anesthetic injection, the procedure itself is largely painless.
After the subcision, patients may undergo additional treatments to further promote collagen generation and scar remodeling. This procedure can be carried out in an outpatient skin clinic setting, unlike laser-based acne scar treatments that rely on thermal disruption of scar tissue and stimulation of collagen.
Subcision can be an effective treatment for acne scars. Research shows that the technique is safe and provides significant long-term improvement for some patients. Subcision can be performed alone by itself, by loosening deep fibrous attachments as the first step, it makes acne scars more amenable to other treatment modalities such as laser as part of a multistep approach. Subcision can also be coupled with other treatments at the same time, to achieve an even better and long-lasting result. Combination techniques are very effective treatments for different types of acne scars and they certainly give more promising results than monotherapy alone.
INNOfill Internal RF Needling is a combined technology of Subcision, Radiofrequency Wave and Filler Injections in one platform, which is an innovative, minimally invasive technique for softening scar tissue and skin tightening.
INNOfill delivers monopolar RF energy through an intradermal needle passed under the skin, thus creating tissue remodeling inside the dermis and hypodermis and stimulates collagen formation and reduces wrinkles. It can combine with dermal filler and restrict filler spread. This combined treatment may prolong the duration and effectiveness of dermal filler treatments and reduce the amount of filler required for maintenance.
In recent years, subcision is usually coupled with injectable fillers as an adjunct. It is aimed at replacing tissue volume as well as stimulating collagen production within the scar, this also essentially acts as a buffer or spacer which stops the bonds from reforming. This can allow the volume of atrophic and depressed scars to be restored, and prevent the scars from being stretched again. For example, when used in combination with fillers such as hyaluronic acid, or collagen-stimulating peptides + hyaluronic acid (HA) skin hydrating agent to smooth out the skin surface, or activators that can repair and activate fibroblast cells such as high-concentration platelet-rich plasma (PRP), it can gradually and naturally rejuvenate the skin. For larger defect areas, autologous fat transfer can be performed simultaneously with subcision. This comprehensive treatment approach will achieve better overall results.
Surgical scar revision is a cosmetic procedure used to improve or reduce the appearance of scars. Scars tend to improve on their own as they mature. It can take 12- 18 months for a scar to mature, but signs that a scar is worsening or not healing properly may require intervention before this time. The major goal of the surgical scar revision at Yan’s is to enhance the appearance of a patient’s skin aesthetically by reducing or eradicating the scar from their body.
Surgical scar revision will improve how you look and feel about yourself. Our goal is to minimize the appearance of your scar to make you more comfortable with your appearance and improve your outlook. We will discuss your scars during a personalised consultation specialised in surgical and minimally invasive scar revision procedures. We will discuss the best approach for your scar and give you realistic expectations on what can be achieved with our state-of-art technologies.
Scar revision procedures are available to treat raised, recessed, obvious or painful scars, unsightly surgical scars, severe ice-pick scars, and blend them with the surrounding skin. A variety of surgical techniques are available for scar revision, including punch excision, direct surgical excision, zigzagplasty, skin flaps, punch grafting and laser surgery, to reorient scar lines. Surgical techniques are often combined with laser treatments to further refine and reduce the appearance of scars and In order to achieve optimal outcomes, more than one procedure may be needed.
Although scar revision surgery does not completely erase scars, it can make scars more cosmetically acceptable. When combined with other advanced technology, revised scars may be nearly imperceptible and provide the best cosmetic outcomes.
Yan’s Group’s medical team has experience in minimally invasive surgery and scar treatment. Doctors assess each scar’s characteristics, considering multiple factors such as scar color, texture, location, thickness, surrounding skin quality, and the patient’s psychological and genetic factors. A tailored repair plan is developed based on the patient’s specific scar conditions to achieve the best expected outcomes. Each individual’s scar situation is different, and the methods of repair vary accordingly. Patients considering scar removal need to have realistic psychological expectations that align with their actual circumstances to be suitable for surgery.
