Acne scars are a common problem that many people face during their adolescent or adult years. They are the result of skin damage caused by acne lesions, which leave behind scarring tissue as they heal. Through a fanning, multi-puncture technique, this method disrupts and loosens the fibrous scar tissue that is pulling down on the epidermis.
This process cuts and separates the fibrous scar tissue anchoring the epidermis downward, allowing the skin surface to return to a more normal position rather than being persistently pulled down.As the tissue then undergoes repair and new collagen production, the appearance of the depressed scar can be improved.

Through a fanning, multi-puncture technique, combined with a threading needle approach, this modified technique can reduce the occurrence of subcutaneous bleeding while still effectively cutting and separating the scar tissue that is pulling down on the epidermis.
This helps to significantly shorten the bruising and recovery time, while maintaining the same therapeutic benefits.
Subcision is not considered a surgical procedure, as it does not require incisions or sutures. The outward appearance will only show a few small needle punctures. It is typically performed under local anesthesia or nerve block, and aside from some initial discomfort during the anesthetic injection, the procedure itself is largely painless.
After the subcision, patients may undergo additional treatments to further promote collagen generation and scar remodeling. This procedure can be carried out in an outpatient skin clinic setting, unlike laser-based acne scar treatments that rely on thermal disruption of scar tissue and stimulation of collagen.
Subcision can be an effective treatment for acne scars. Research shows that the technique is safe and provides significant long-term improvement for some patients. Subcision can be performed alone by itself, by loosening deep fibrous attachments as the first step, it makes acne scars more amenable to other treatment modalities such as laser as part of a multistep approach. Subcision can also be coupled with other treatments at the same time, to achieve an even better and long-lasting result.
